What a good evaluation template covers
Most hockey evaluation forms, whether they're used at tryouts or by a scout following a player, cover the same four areas: skating, puck skills, hockey sense, and compete level. Many add position-specific rows, like faceoffs for centers or gap control for defensemen.
The weak spot is that almost everything on those forms is a 1-to-5 rating filled in from memory after the game. Ratings are useful, but they're only as good as what the evaluator remembers — and people remember the last shift and the one big mistake.
Count during the game, rate after it

Keep notes short and specific
